people who say "no offense" always offend me
do they mean to cause offense or is it just careless language? either way, it's still a thing that bugs me

Does anyone else think it's a bit cowardly to suggest it's "just careless" when language can be so hurtful? Isn't calling it something shouldn't be as simple as an honest mistake?
